What erythritol is, and how it is made

Erythritol is a sugar alcohol, listed in Europe as E968. You will often read that it occurs naturally in pears, grapes and mushrooms. True, but nobody makes it that way. There is far too little to harvest.

The commercial process is fermentation. Producers take glucose from corn or wheat starch and feed it to a yeast, usually Moniliella pollinis or Moniliella megachiliensis, then purify and dry what the yeast produces. That is a controlled industrial process in a steel tank. Calling it natural is a stretch, though calling it artificial is unfair too. Fermentation is an old technique. Beer is fermented. The honest word is manufactured.

Good to know

Erythritol earns its popularity. It is about 70% as sweet as sugar, contributes almost no calories, does not raise blood glucose, does not feed the bacteria that cause tooth decay, and crystallises like sugar so it behaves properly in baking. It is also cheap. Those are real advantages and we are not going to pretend otherwise.

Where each one ends up in your body

Here is the part most comparisons skip. Erythritol is a very small molecule, small enough to pass straight through your intestinal wall. Studies put absorption somewhere between 60 and 90% depending on the dose. It enters your bloodstream, circulates, and because humans have no enzyme to break it down, your kidneys filter it out and send it to your urine unchanged. Around 80 to 90% is gone within a day or two.

That is normally presented as a virtue, and in one sense it is. Bypassing the colon is exactly why erythritol upsets fewer stomachs than xylitol or sorbitol. But it means a compound your body cannot use is circulating in your blood for a day or more after dessert.

A fair question, and an honest answer

Is that a problem? We do not know, and anyone who tells you they do is overselling. Your body already makes small amounts of erythritol from glucose on its own, so blood erythritol is not purely a record of what you ate. The measured question is whether the much higher levels that come from eating it matter, and that is exactly what the research below has been arguing about since 2023.

What happens with monk fruit

Mogrosides, the sweet compounds in monk fruit, behave differently. Your body does not absorb and circulate them the way it does erythritol, and it does not break them down for energy. That is why a serving is about 1 kcal and has no glycemic index per portion. There is no equivalent debate about mogrosides in your bloodstream, because they largely do not get there.

Why monk fruit gets blamed for erythritol

If you have read that monk fruit gave someone bloating or a strange cooling aftertaste, there is a good chance erythritol did it. Most products sold abroad as monk fruit sweetener are mostly erythritol with a small amount of concentrated mogrosides added for sweetness. The fruit gets top billing on the front and a minor role in the bag.

What the heart research actually found

In February 2023, a team at the Cleveland Clinic published in Nature Medicine. They screened blood from 1,157 patients being assessed for heart risk and found erythritol near the top of the list of compounds linked to heart attack, stroke and death over three years. They then checked two more groups, 2,149 people in the United States and 833 in Europe. In both, the quarter with the highest blood erythritol were roughly twice as likely to have one of those events as the quarter with the lowest. The adjusted figures were 1.80 and 2.21. In the laboratory, erythritol made platelets clump more readily, and eight healthy volunteers who drank 30 g held raised blood levels for more than two days.

Now the objections, which are serious

Everyone in those groups was already at a cardiologist. That is not the general population. More important, your body produces erythritol itself from glucose, and it produces more of it when blood sugar control is poor. So a high reading may be a symptom of an existing metabolic problem rather than a record of dessert. Several researchers have argued exactly that. A separate four-week trial gave 24 people with type 2 diabetes 36 g a day and reported improved blood vessel function, which points the opposite way. A two-year rat study at far higher doses found no effect on survival or cancer rates.

So the fair summary is this. An association has turned up repeatedly in people already at risk, with a plausible mechanism behind it, and nobody has yet shown that eating erythritol causes the events. European regulators re-evaluated it in 2023 and kept it approved. If you eat erythritol, this is not a reason to panic. It is a reason to notice that the safety file is open, which is worth knowing when you are choosing between two products.

How to choose between them

They are better at different jobs. Most kitchens have room for a considered answer rather than a winner.

  1. Baking a cake? Erythritol has the advantage
