Sucralose starts as sugar and ends up as something your body does not recognise. The research on it is genuinely interesting, and almost every headline about it gets one detail wrong.

Sucralose is the most successful artificial sweetener ever made. It is in diet drinks, protein powders, sauces, medicines and thousands of products labelled sugar free, largely because it is cheap, intensely sweet and does not fall apart in an oven.

It also attracts more alarming headlines than any other sweetener. Some of them point at real research. Some of them describe studies that were not about sucralose at all. We sell monk fruit, so we went through the three loudest claims one at a time and cut the one that does not hold up.

What sucralose is, and how it is made

Sucralose starts as ordinary sugar. Chemists then replace three hydroxyl groups on the molecule with three chlorine atoms. The result looks a little like sugar, tastes about 600 times sweeter, and does not fit the enzymes your body uses to break sugar down.

That last part is the whole point. Because your digestive system cannot process it, sucralose contributes almost no calories. Most of what you swallow passes through unabsorbed, and the fraction that does get absorbed is filtered out by your kidneys.

Made from sugar is doing a lot of work

The marketing line for years was that sucralose is made from sugar, so it tastes like sugar. Technically accurate. Chlorine gas is also made from salt. What matters is what comes out of the process, and what comes out is a chlorinated compound that does not occur anywhere in nature and that nobody ate before the 1990s.

Why manufacturers love it

It is heat stable, which most artificial sweeteners are not. Aspartame collapses in an oven. Sucralose survives, so it works in baked goods, sauces and anything pasteurised. Add a very low price and 600 times the sweetening power of sugar and you can see why it ended up everywhere.

Worth knowing before the next section. Sucralose has been approved by every major food regulator, including in Europe, and none of them has withdrawn or restricted it. Everything below sits inside that context.

Three claims, checked one by one

We looked up each of these and read what the studies actually measured. One holds up with qualifications, one is real but points somewhere different from where people aim it, and one falls apart.

Claim one: it damages DNA. This traces to a 2023 paper by Susan Schiffman and colleagues in the Journal of Toxicology and Environmental Health. The finding is narrower than the headlines. The genotoxic compound they identified was not sucralose itself but sucralose-6-acetate, an impurity left over from manufacturing, found at up to 0.67% in commercial samples, which rodent work suggests may also form in the gut after eating sucralose. In laboratory dishes it broke up DNA in human blood cells, and both compounds made gut tissue more permeable. The researchers noted the trace amount in one daily sweetened drink could exceed the European threshold for genotoxic substances.

The qualifications on claim one, which matter

Every experiment was done on cells in dishes, not in people. And this study is contested to an unusual degree: the maker of Splenda began legal proceedings against the lead author in 2023, arguing the claims were false and that their product contains no detectable sucralose-6-acetate. We are not in a position to settle that, and we are not going to pretend it is settled. Treat it as a serious open question rather than a finding.

Claim two: it is linked to Alzheimer’s

The study people mean is usually the 2017 Framingham analysis in Stroke, which found that people drinking artificially sweetened soft drinks daily had roughly three times the risk of Alzheimer’s dementia. That is a real result in a respected cohort. It is also not a study about sucralose. It measured artificially sweetened drinks as a single category, and the diet questionnaires ran from 1991 to 2001, when sucralose had only just been approved and was barely in the food supply. Whatever those participants were drinking, it was mostly aspartame and saccharin. More recent cohort work has included sucralose alongside other sweeteners and found associations with faster cognitive decline, but these remain observational studies that cannot separate the drink from the person drinking it.

Claim three: it crosses the blood-brain barrier

We could not find research supporting this, so we are not printing it. What does exist is more interesting anyway. A 2025 randomised trial in Nature Metabolism gave 75 adults a sucralose drink, a sugar drink or water, and scanned their brains. Sucralose increased blood flow to the hypothalamus, the region that governs appetite, and left people hungrier than the sugar drink did. That is not the sweetener getting into the brain. That is the brain reacting to sweetness that arrives without the calories it was expecting.

History

Eight centuries of eating it

Monk fruit has been grown and brewed in southern China since roughly the thirteenth century, mostly as a tea for coughs and sore throats. That is a long record of people eating something and not coming to obvious harm. It is history rather than evidence, and we will not dress it up as a clinical result.

What the human studies show

A 2025 systematic review in Nutrients pooled five randomised controlled trials of monk fruit extract and reported lower post-meal glucose and insulin responses with no serious adverse effects. Those trials used concentrated extract rather than our decoction, so read them as promising rather than as being about this bottle.

The mogroside research

Studies point to antioxidant and anti-inflammatory activity in mogrosides, the sweet compounds in the fruit. This work is preclinical, mostly in cells and animals, and none of it is a health promise. Monk fruit is a food, not a medicine, and we are not allowed to say otherwise for good reason.

The catch

A shorter file, not a perfect one

No safety signals have turned up in the toxicology published so far. Monk fruit has also been studied far less than sucralose, which has decades of scrutiny behind it. A clean record partly reflects a smaller pile of research, and pretending otherwise would be the same trick we just criticised.

Why the recognition argument is worth something

There is a version of the natural argument that is lazy, and a version that holds. The lazy one says natural is safe, which is nonsense, since plenty of plants will kill you. The version that holds is narrower and it is about familiarity.

What your body has seen before

Monk fruit is a fruit that humans have eaten for eight hundred years. Your digestive system has met this class of compound repeatedly across that time, and the ways it handles them are old and well worn. Sucralose is a chlorinated molecule invented in 1976 that appears nowhere in nature. Your body has no established route for it, which is exactly why it passes through mostly untouched.

Where that argument stops

Passing through untouched is arguably a point in sucralose’s favour, not against it. An ingredient your body ignores completely cannot do much to you. The honest summary is that long dietary use lowers the odds of an unpleasant surprise. It does not eliminate them, and it is a reason for mild preference rather than alarm.

That is why our position on sucralose is not that it is dangerous. It is approved, it is enormously well studied, and no regulator has moved against it. Our position is that the questions attached to it keep arriving and none of them has been closed, while the questions attached to a boiled fruit have not arrived at all. Given the choice, and where you actually have a choice, we know which we would rather drink.

How to choose between them

Five practical notes, including the situations where sucralose is the better tool.

  1. For baking, sucralose has a real advantage

    It survives heat and it is cheap, which is why so many sugar-free products use it. Our powder bakes because the tapioca fibre gives it bulk, but a liquid decoction cannot replace what sugar does structurally.

  2. For drinks, monk fruit is the easier choice

    A few drops in tea, warm milk or yogurt. Nothing here needed the sweetener to do anything except taste sweet. Avoid high-acid drinks, where any aftertaste comes forward.

  3. If you are hungrier after diet drinks, that is worth noticing

    The 2025 hypothalamus trial found sucralose left people hungrier than a sugar drink did. If you have noticed that pattern in yourself, it is not imagination, and switching what you sweeten with is a cheap experiment.

  4. Check whether you actually have a choice

    In a medicine, a protein powder or one specific soft drink, often you do not. That is fine. Spend the attention on the things you control, which is mostly your own kitchen.

  5. Judge our aftertaste honestly

    Ours is fruity and mild and it fades. Some people like it straight away and some never do. Try it in warm milk before you decide, because that is where it shows itself most kindly.

One honest tip: if a claim about a sweetener sounds frightening, check whether the study was done in people or in a dish. That single question sorts most of what you will read online.
